This program provides services at its centers if you need help taking care of yourself during the day due to a chronic condition. If you are caring for another person who has a chronic condition, the center can provide you with education, support, and respite care to get breaks. If you have or are caring for someone with a chronic condition like Alzheimer’s disease, Parkinson’s disease, or diabetes, you may be able to get help from this program.
About this program
This program provides services at its centers if you need help taking care of yourself during the day due to a chronic condition. You can participate in social activities, have your health monitored, get physical therapy, and learn how to take better care of yourself. The program also offers meals and transportation to and from the center.
If you are caring for another person who has a chronic condition, the center can provide you with education, support, and respite care to get breaks. A case manager can work with you to select services for the person in your care. You can also take part in support groups and one-on-one sessions with a professional.
