You can get services to help you stay active and healthy. You and your caregiver can take part in this program if you have been diagnosed with or are being treated for Alzheimer’s disease or other related conditions.

This program can help you continue living at home instead of moving to a nursing home or other facility if you have Alzheimer’s disease or a related illness. You can get services to help you stay active and healthy. A care plan is created to provide services for your needs and can include health care, home care, and social activities.
If you are a caregiver, you can also get support and services from this program, such as education and training about Alzheimer’s as well as respite care to provide you with short breaks for rest. Respite care can be any time during the day or in the evening hours.
