- Inpatient and outpatient hospital services
- Emergency room visits
- Lab tests and x-ray services
- Medical supplies, equipment, and prostheses
- Physical, occupational, hearing, and speed therapy
- Medical transportation
- Nursing home care
- Hospice care
- Mental health services
- Home health agency visits
- Family planning services
To get benefits, you should go to a health care provider that is part of the program’s network.

To apply, fill out the online application. You may also print and fill out the application form and mail it to your local Medicaid office.
