This program gives you health care and support services so that you can safely stay in your own home. You can take part in this program if you have limited income, need help caring for yourself on a daily basis, and are 60 or older or are between the ages of 18 and 59 and have a disablity.
About this program
This program can help you take care of your everyday needs by providing some or all of the following services:
- Case management: A social service professional will work with you and your family to decide which services you will need so that you can stay at home.
- Homemaking: This includes help with housework, laundry, shopping, and running errands.
- Personal care: A service professional will help you with your daily activities such as bathing, dressing, and grooming.
- Meals: You can get home-delivered meals and help with preparing meals at home.
