State
This program helps you to continue living at home by providing case management, information and referral, and crisis intervention. You can get help from this program if you have age-related functional disabilities and are 65 years of age or older. You can also take part in this program if you are younger than 65 years of age, have a disability that is not due to age, and have limited income.
About this program
This program helps you to continue living at home by providing the following services:
- Case management: A social worker will work with you to make an assessment of your needs and resources and will come up with a plan of service that will allow you to continue living independently. If it is determined that you may need institutional care, the program will help you find a place.
- Information and referral: You will be referred to another agency if the program determines that your needs are best served by another agency.
- Crisis intervention: This is a 60-day service that can help you during a crisis or provide guidance through a change in your life.
