Common questions
- Can I connect my CentraCare records to Eleplan?
- Yes. If you have a patient portal account with CentraCare, you can search for CentraCare in Eleplan, sign in to the portal, and bring your records into your care plan.
- Does Eleplan see my patient portal password?
- No. You sign in on the patient portal's own page, and it gives Eleplan access to your records through the standard patient-access process (SMART on FHIR). Your password is never shared with Eleplan.
- Is Eleplan affiliated with CentraCare?
- No. Eleplan is an independent company and is not affiliated with CentraCare. Federal rules give patients the right to their own records and to share them with the apps they choose, and that is how this connection works.
